WhiteBoard style discussion on a newer theory that is gaining attention on the bradykinin pathway and how it may contribute to the respiratory failure seen with COVID-19. This pathway involves a bradykinin metabolite that typically is degraded by ACE2. When SARS-CoV-2 infects cells using the ACE2R, ACE2 is down regulated. As such, there is less of it available to convert the bradykinin metabolite DABK to it's more benign form. DABK then leads to inflammation, vascular permeability, and vasodilation that contributes to pulmonary alveolar fluid build up, impaired gas exchange, and inflammatory changes in the lungs. It also stimulates more production of bradykinin! Check out this video to understand how this may work. In this video, we will discuss this mechanism as it relates to COVID using various depictions. We will also discuss how the discovery of this pathway may lead to the identification of new therapeutic targets!
